MFD Membership Payment
All MFD Membership fees are due and payable at the time of registration and are non-refundable when services have been provided. All patient portions for services received are DUE IN FULL AT TIME OF SERVICE in order to receive the advantages of the MFD membership and savings. Your plan duration is for one year from your registration date.
To qualify for 10% savings, payment must be made with cash, check, or a major credit card. If payment is made with a third-party lender, such as Care Credit, there will only be a 5% savings on general and major dental procedures and a 5% savings on limited orthodontics.
The MFD Membership is non-transferable and non-refundable. This cannot be combined with dental benefits. If you obtain dental benefits during the membership period, the membership will be null and void.
